WechselwirkungenExploiting protein homeostasis is a new therapeutic approach in cancer. Nelfinavir (NFV) is an HIV protease inhibitor that induces endoplasmic reticulum (ER) stress in cancer cells. Under conditions of ER stress, misfolded proteins are transported from the ER back to the cytosol for subsequent degradation by the ubiquitin-proteasome system. Bortezomib (BZ) is a proteasome inhibitor and interferes with degradation of misfolded proteins. Here, we show that NFV and BZ enhance proteotoxicity in non-small cell lung cancer (NSCLC) and multiple myeloma (MM) cells. The combination synergistically inhibited cell proliferation and induced cell death. Activating transcription factor (ATF)3 and CCAAT-enhancer binding protein homologous protein (CHOP), markers of ER stress, were rapidly increased, and their siRNA-mediated knockdown inhibited cell death. Knockdown of double-stranded RNA activated protein kinase-like ER kinase, a signal transducer in ER stress, significantly decreased apoptosis. Pretreatment with the protein synthesis inhibitor, cycloheximide, decreased levels of ubiquitinated proteins, ATF3, CHOP, and the overall total cell death, suggesting that inhibition of protein synthesis increases cell survival by relieving proteotoxic stress. The NFV/BZ combination inhibited the growth of NSCLC xenografts, which correlated with the induction of markers of ER stress and apoptosis. Collectively, these data show that NFV and BZ enhance proteotoxicity in NSCLC and MM cells, and suggest that this combination could tip the precarious balance of protein homeostasis in cancer cells for therapeutic gain.

Anwendungen & Verwendung

Beschreibung

Viracept was launched in the US as an orally-available, non-peptidic HIV protease inhibitor. It is prepared in an eight step synthesis beginning with N- (benzyloxycarbonyl)-L-serine β-lactone. The HIV protease inhibitory activity blocks the processing of gag and gagpol polyproteins that are required for viral maturation. It was discovered based on an experimentally derived 3D-structure of HIV-1 protease. Viracept has a high oral bioavailability, lacks toxicity, and has a resistance profile different from other protease inhibitors. Viracept with ziduvudine and larnivudine generated a 98% mean reduction from baseline in viral load after 24 weeks compared to ziduvudine and larnivudine alone. Viracept also had better CD4 counts and is available in adult and pediatric formulations.

Verwenden

Nelfinavir Mesylate is an antiviral. HIV protease inhibitor. It is a COVID19-related research product.

Competition for the cytochrome p450 enzyme CYP3A by nelfinavir may inhibit the metabolism of these medications /amiodarone, astemizole, cisapride, ergot derivatives, midazolam, quinidine, terfenadine, triazolam/ and create the potential for serious and/or life threatening cardiac arrhythmias or prolonged sedation
concurrent use is not recommended.
Concurrent use /with lamivudine/ causes a 10% increase in the AUC of lamivudine.
Concurrent administration /of oral contraceptives such as: ethinyl estradiol or norethindrone/ with nelfinavir causes a decrease in the plasma concentrations of these medications
alternate or additional contraceptive measures should be used.

Human Toxicity Excerpts/ALTERNATIVE and IN VITRO TESTS/ ... In the present work, ...performed a comparative analysis of the spectrum of action of HIV PIs on different Leishmania spp., including strains obtained from HIV-positive patients receiving or not receiving antiretroviral treatment. The effects of nelfinavir and saquinavir on Leishmania proliferation were assessed by means of a colorimetric assay (MTT). Subsequently, the effect of nelfinavir on aspartic peptidase activity from Leishmania spp. was assessed by following the degradation of the fluorogenic substrate MCA-G-K-P-I-L-F-F-R-L-K-DNP-Arg-NH(2). Nelfinavir was capable of significantly reducing the multiplication of many Leishmania reference strains and isolates obtained from HIV-positive patients receiving or not receiving antiretroviral treatment. Leishmania major growth was inhibited by ?50%, while all other flagellates were strongly inhibited (at least 94%), except for a Leishmania chagasi strain obtained from an HIV-positive patient under treatment with highly active antiretroviral therapy (HAART). Culture of this isolate in the presence of nelfinavir induced a considerable reduction in the aspartic peptidase activity. In addition, nelfinavir was also capable of inhibiting the aspartic peptidase activity of all Leishmania strains tested. The present data contribute to the study of the effect of HIV PIs on Leishmania infection and add new insights into the possibility of exploiting aspartic peptidases as promising targets in order to generate novel medications to treat leishmaniasis./OTHER TOXICITY INFORMATION/ Competition for the cytochrome p450 enzyme CYP3A by nelfinavir may inhibit the metabolism of these medications /amiodarone, astemizole, cisapride, ergot derivatives, midazolam, quinidine, terfenadine, triazolam/ and create the potential for serious and/or life threatening cardiac arrhythmias or prolonged sedationconcurrent use is not recommended./GENOTOXICITY/ No chromosome aberrations were detected in human lymphocyte assays.

Non-Human Toxicity Excerpts/LABORATORY ANIMALS: Acute Exposure/ ... The aim of this study was to evaluate the effects of nelfinavir (NFV), an HIV PI, and of alanyl-glutamine (AQ) supplementation, on intestinal cell migration, proliferation, apoptosis and necrosis, using IEC-6 cells and on intestinal crypt depth, villus length, villus area, mitotic index and apoptosis in Swiss mice. Migration was evaluated at 12 and 24hr after injury using a wound healing assay. Cellular proliferation was measured indirectly at 24 and 48hr using tetrazolium salt WST-1. Apoptosis and necrosis were measured by flow cytometry using the Annexin V assay. Intestinal morphometry and mitotic index in vivo were assessed following a seven-day treatment with 100mg/kg of NFV, given orally. In vivo proliferation and apoptosis were evaluated by intestinal crypt mitotic index and immunohistochemistry, respectively. In vitro, AQ supplementation enhanced IEC-6 cell migration and proliferation, following challenge with NFV. In vivo, AQ increased intestinal villus length, villus area, crypt depth and cell proliferation and cell migration, following treatment with NFV. AQ did not decrease cell death induced by NFV both in vivo and in vitro. AQ supplementation is potentially beneficial in preventing the effects of PIs, such as NFV, in the intestinal tract./LABORATORY ANIMALS: Chronic Exposure or Carcinogenicity/ ... Researchers conducted a pharmacological profiling of seven genotypically distinct breast cancer cell lines using a subset of inhibitors of breast cancer cells from a screen of the Johns Hopkins Drug Library. To identify molecular targets of nelfinavir, identified in the screen as a selective inhibitor of HER2-positive cells, conducted a genome-wide screen of a haploinsufficiency yeast mutant collection. Researchers evaluated antitumor activity of nelfinavir with xenografts in athymic nude mouse models (n = 4-6 per group) of human breast cancer and repeated mixed-effects regression analysis. All statistical tests were two-sided. Results Pharmacological profiling showed that nelfinavir, an anti-HIV drug, selectively inhibited the growth of HER2-positive breast cancer cells in vitro. A genome-wide screening of haploinsufficiency yeast mutants revealed that nelfinavir inhibited heat shock protein 90 (HSP90) function. Further characterization using proteolytic footprinting experiments indicated that nelfinavir inhibited HSP90 in breast cancer cells through a novel mechanism. In vivo, nelfinavir selectively inhibited the growth of HER2-positive breast cancer cells (tumor volume index of HCC1954 cells on day 29, vehicle vs nelfinavir, mean = 14.42 vs 5.16, difference = 9.25, 95% confidence interval [CI] = 5.93 to 12.56, P < .001tumor volume index of BT474 cells on day 26, vehicle vs nelfinavir, mean = 2.21 vs 0.90, difference = 1.31, 95% CI = 0.83 to 1.78, P < .001). Moreover, nelfinavir inhibited the growth of trastuzumab- and/or lapatinib-resistant, HER2-positive breast cancer cells in vitro at clinically achievable concentrations. Nelfinavir was found to be a new class of HSP90 inhibitor and can be brought to HER2-breast cancer treatment trials with the same dosage regimen as that used among HIV patients./LABORATORY ANIMALS: Developmental or Reproductive Toxicity/ Reproductive studies in rats or rabbits at nelfinavir exposure levels calculated to be equivalent to or substantially less than the recommended human dosage based on measurements of steady-state area under the plasma concentration-time curve (AUC) have not revealed evidence of embryotoxicity or teratogenicity. There were no treatment-related effects on fetal development or on maternal toxicity in rats at nelfinavir exposure levels calculated to be equivalent to the recommended human dose. Reproductive studies in rats that involved administering nelfinavir from mid-pregnancy through lactation at exposure levels calculated to be equivalent to the recommended human dose have not revealed adverse effects on survival, growth, or development of offspring to weaning. While a slight decrease in maternal weight was observed in rabbits exposed to nelfinavir levels substantially less than the recommended human dose, there were no treatment-related effects on fetal development./LABORATORY ANIMALS: Developmental or Reproductive Toxicity/ There was no evidence that nelfinavir affected fertility, mating, or embryo survival in female rats or mating in male rats when the drug was given at doses providing exposure levels calculated to be equivalent to those provided by the recommended human dose. In addition, reproductive performance in offspring was not affected by maternal exposure to nelfinavir.
