TreatmentAntidotes to cyanide poisoning include hydroxocobalamin and sodium nitrite, which release the cyanide from the cytochrome system, and rhodanase, which is an enzyme occurring naturally in mammals that combines serum cyanide with thiosulfate, producing comparatively harmless thiocyanate. Oxygen therapy can also be administered. (L97)
Gesundheitliche AuswirkungenExposure to high levels of cyanide for a short time harms the brain and heart and can even cause coma, seizures, apnea, cardiac arrest and death. Chronic inhalation of cyanide causes breathing difficulties, chest pain, vomiting, blood changes, headaches, and enlargement of the thyroid gland. Skin contact with cyanide salts can irritate and produce sores. (L96, L97)
Unerwünschte WirkungenLacrimator (Lachrymator) - A substance that irritates the eyes and induces the flow of tears.

ToxizitätsübersichtIDENTIFICATION AND USE: This chemical is a gray to brown powder which is soluble in ketones and esters. It is used in polymers and adhesive systems, as a high strength backbone or cross linking intermediate. HUMAN EXPOSURE AND TOXICITY: Isocyanates are irritating to the skin and the mucous membranes, Skin conditions range from localized itching to wide spread eczema. Eye affections are less common and lacrimation is often found, conjunctivitis is rare. Respiratory system effects can be severe. ANIMAL STUDIES: Limited evidence of carcinogenicity in animals. Groups of male and female B6C3F1 mice, six weeks old were given dietary concentrations of this chemical for 78 weeks followed by a basal diet for a further 25 weeks. There was no significant difference in treated and control animals. There was no increased incidence of tumors in treated animals. Groups of male and female Fischer 344/N rats, six weeks of age received the chemical by gavage for 22 weeks. Animals were subsequently fed a basal diet for 22 weeks. A significant increase in combined incidence of leukemias and malignant lymphomas was observed. Increased incidence of various skin tumors was also noted on the head, back and inguinal areas in males and females. Increased incidences of Zymbal gland tumors was observed. Levels of Evidence of Carcinogenicity: Male Rats: PositiveFemale Rats: PositiveMale Mice: NegativeFemale Mice: Negative.Cyanide is an inhibitor of cytochrome c oxidase in the fourth complex of the electron transport chain (found in the membrane of the mitochondria of eukaryotic cells). It complexes with the ferric iron atom in this enzyme. The binding of cyanide to this cytochrome prevents transport of electrons from cytochrome c oxidase to oxygen. As a result, the electron transport chain is disrupted and the cell can no longer aerobically produce ATP for energy. Tissues that mainly depend on aerobic respiration, such as the central nervous system and the heart, are particularly affected. Cyanide is also known produce some of its toxic effects by binding to catalase, glutathione peroxidase, methemoglobin, hydroxocobalamin, phosphatase, tyrosinase, ascorbic acid oxidase, xanthine oxidase, succinic dehydrogenase, and Cu/Zn superoxide dismutase. Cyanide binds to the ferric ion of methemoglobin to form inactive cyanmethemoglobin. (L97)
Signs and SymptomsCyanide poisoning is identified by rapid, deep breathing and shortness of breath, general weakness, giddiness, headaches, vertigo, confusion, convulsions/seizures and eventually loss of consciousness. (L96, L97)

Antidote and Emergency TreatmentNoncardiogenic pulmonary edema and bronchospasm are the most immediate serious clinical consequences of isocyanate exposure. Markedly symptomatic patients should receive oxygen, ventilatory support, and an intravenous line. Treatment for asthma includes inhaled sympathomimetics (salbutamol, metaproterenol), intravenous theophylline, parenteral sympathomimetics (epinephrine, terbutaline), and steroids. /Isocyanates/Most treatment is symptomatic. Mydriatics, systemic analgesics, and topical antibiotics (Sulamyd) may be used for corneal abrasions. There is no effective therapy for sensitized workers, and these people should be moved to a work site devoid of exposure to isocyanates. /Isocyanates/
