Used for the treatment of corticosteroid-responsive inflammation of the palpebral and bulbar conjunctiva, cornea and anterior segment of the globe. It has a role as an anti-inflammatory drug. It is a tertiary alpha-hydroxy ketone, a glucocorticoid, a 17alpha-hydroxy steroid, an 11beta-hydroxy steroid, a 20-oxo steroid, a fluorinated steroid and a 3-oxo-Delta(1),Delta(4)-steroid. It is functionally related to a Delta(1)-progesterone.

Corticosteroids such as fluorometholone inhibit the inflammatory response to a variety of inciting agents and probably delay or slow healing. They inhibit the edema, fibrin deposition, capillary dilation, leukocyte migration, capillary proliferation, fibroblast proliferation, deposition of collagen, and scar formation associated with inflammation.
There is no generally accepted explanation for the mechanism of action of ocular corticosteroids. However, corticosteroids are thought to act by the induction of phospholipase A2 inhibitory proteins, collectively called lipocortins. It is postulated that these proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid. Arachidonic acid is released from membrane phospholipids by phospholipase A2. Their primary target is the cytosolic glucocorticoid receptor. After binding the receptor the newly formed receptor-ligand complex translocates itself into the cell nucleus, where it binds to many glucocorticoid response elements (GRE) in the promoter region of the target genes. The DNA bound receptor then interacts with basic transcription factors, causing the increase in expression of specific target genes.

FLUOROMETHOLONE
Established Pharmacologic Class [EPC] - Corticosteroid
Mechanisms of Action [MoA] - Corticosteroid Hormone Receptor Agonists
Fluorometholone is a Corticosteroid. The mechanism of action of fluorometholone is as a Corticosteroid Hormone Receptor Agonist.
FLUOROMETHOLONE
Substances that reduce or suppress INFLAMMATION.
A group of CORTICOSTEROIDS that affect carbohydrate metabolism (GLUCONEOGENESIS, liver glycogen deposition, elevation of BLOOD SUGAR), inhibit ADRENOCORTICOTROPIC HORMONE secretion, and possess pronounced anti-inflammatory activity. They also play a role in fat and protein metabolism, maintenance of arterial blood pressure, alteration of the connective tissue response to injury, reduction in the number of circulating lymphocytes, and functioning of the central nervous system.

Agents that are used to treat allergic reactions. Most of these drugs act by preventing the release of inflammatory mediators or inhibiting the actions of released mediators on their target cells. (From AMA Drug Evaluations Annual, 1994, p475)
UsesFluorometholone is a glucocorticoid; anti-inflammatory.
General DescriptionFluorometholone, 9-fluoro-11β,17-dihydroxy-6α-methylpregn-4-ene-3,20-dione(Fluor-Op, FML), lacks the typical C21 OH group of GCsand is used exclusively in ophthalmic products. The 17-acetateof fluorometholone is also used as an ophthalmic suspension(Flarex).
Preparation1.9,11-Epoxy Reaction Add 10g of 6-methyl-17-hydroxy-1,4,9-trien-pregn-3,20-dione and 120ml of acetone to a reaction flask, stir, cool to 0℃, add 9g of NBS over 30 minutes, maintain the reaction at 5-10℃ for 2 hours, add 10% sodium carbonate aqueous solution to neutralize to pH=6.5, raise the temperature to 20±2℃, add 15ml of 10% sodium hydroxide aqueous solution over 1 hour, maintain the temperature at 20-25℃ for 2 hours, neutralize to pH=7 with acetic acid, concentrate under reduced pressure until there is no acetone odor, dilute to ice water, filter, dry to obtain 11.2g of 9,11-epoxide. 2.9,11-Ring-opening reaction Add 11.2g of 9,11-epoxide obtained from the reaction of 9,11-epoxide and 60ml of DMF to a reverse flask, stir, cool to -5℃, introduce hydrogen fluoride gas, maintain the reaction at -5~0℃ for 1 hour, dilute in ice water, adjust the pH to 7 with ammonia, filter, dry, and obtain 9.5g of fluorometholone. Recrystallize in acetone to obtain 8.5g of fluorometholone, MP: 290-300℃. Refining: Add 100g of crude fluorometholone (97%) to 400mL of N,N-dimethylformamide, stir and heat to 80-90℃, maintain for 0.5h, until completely dissolved, then slowly add 1L of anhydrous methanol, maintaining the temperature within the range of 60-70℃ throughout the addition process. After addition, maintain for 0.5h, then cool to 0-5℃ and maintain at 0-5℃ for crystallization for 12h. Filter to obtain off-white crystalline fluorometholone concentrate, dry to obtain 30g of fluorometholone concentrate (purity 99.55%, known single impurities 0.14%, total impurities 0.09%).
