概要

Nystatin is a polyene antifungal drug that has broad-spectrum fungicidal and fungistatic activity against a number of yeasts and fungi, most notably Candida species. It is one of the most effective antifungal agents synthesized by bacteria, in this case a strain of Streptomyces noursei, and is closely related to [amphotericin B], differing only slightly in structure. Nystatin has a greater antifungal activity than amphotericin B - parenterally administered nystatin, however, is associated with significant toxicity and is not available in a formulation appropriate for systemic use. As it undergoes very little absorption following oral or topical administration, nystatin's efficacy is limited to the treatment/prevention of cutaneous, mucocutaneous, and gastrointestinal fungal infections.

Health EffectsTachycardia, bronchospasm, facial swelling, and non-specific myalgia have also been rarely reported. Stevens-Johnson syndrome has been reported very rarely. (L1873)
Hepatotoxicity · 1 件

Nystatin therapy has been associated with a low rate of serum enzyme abnormalities, although it has been difficult to attribute these elevations to nystatin. Despite its use for several decades, there have been no convincing cases of acute hepatic injury linked to nystatin therapy. While nystatin is usually is not normally absorbed, low concentrations may enter the circulation in patients with inflammation and damage to the gastrointestinal tract. Nevertheless, nystatin is considered very safe and is unlikely to cause hepatic injury.

Environmental Fate · 3 件

TERRESTRIAL FATE: Based on a classification scheme(1), an estimated Koc value of 170(SRC), determined from a water solubility of 3.60X10+2 mg/L(2) and a regression-derived equation(3), indicates that nystatin is expected to have moderate mobility in soil(SRC). Volatilization of nystatin from moist soil surfaces is not expected to be an important fate process(SRC) given an estimated Henry's Law constant of 2.0X10-7 atm-cu m/mole(SRC), using a fragment constant estimation method(4). Nystatin is not expected to volatilize from dry soil surfaces(SRC) based upon an estimated vapor pressure of 8.68X10-7 mm Hg(SRC), determined from a fragment constant method(5). A 4% degradation in 28 days using the Closed Bottle test(6) indicates that biodegradation is not an important environmental fate process in soil.

AQUATIC FATE: Based on a classification scheme(1), an estimated Koc value of 170(SRC), determined from a water solubility of 3.60X10+2 mg/L(2) and a regression-derived equation(3), indicates that nystatin is expected to adsorb slightly to suspended solids and sediment(SRC). Volatilization from water surfaces is not expected(3) based upon an estimated Henry's Law constant of 2.0X10-7 atm-cu m/mole(SRC), developed using a fragment constant estimation method(4). According to a classification scheme(5), an estimated BCF of 22(SRC), from its water solubility and a regression-derived equation(6), suggests the potential for bioconcentration in aquatic organisms is low(SRC). A 4% degradation in 28 days using the Closed Bottle test(7) indicates that biodegradation is not an important environmental fate process in water.

ATMOSPHERIC FATE: According to a model of gas/particle partitioning of semivolatile organic compounds in the atmosphere(1), nystatin, which has an estimated vapor pressure of 8.7X10-7 mm Hg at 25 °C(SRC), determined from a fragment constant method(2), will exist in both the vapor and particulate phases in the ambient atmosphere.Vapor-phase nystatin is degraded in the atmosphere by reaction with photochemically-produced hydroxyl radicals(SRC); the half-life for this reaction in air is estimated to be 1.5 hours(SRC), calculated from its rate constant of 2.5X10-10 cu cm/molec-sec at 25 °C(SRC), that was derived using a structure estimation method(3). Vapor-phase nystatin is degraded in the atmosphere by reaction with ozone(SRC); the half-life for this reaction in air is estimated to be 2.6 hours(SRC), calculated from its rate constant of 1.1X10-16 cu cm/molec-sec at 25 °C(SRC), that was derived using a structure estimation method(4). Particulate-phase nystatin may be removed from the air by wet or dry deposition(SRC). The rate of nystatin decomposition is affected by light(5) and therefore the compound may be susceptible to direct photolysis by sunlight.

Exposure RoutesOral, topical(intravaginal). Nystatin is not absorbed from intact skin or mucous membrane.
Toxicity Summary · 1 件

Nystatin interacts with 14-α demethylase, a cytochrome P-450 enzyme necessary for the conversion of lanosterol to ergosterol. This results in inhibition of ergosterol synthesis and increased fungal cellular permeability. Nystatin exerts its antifungal activity by binding to ergosterol found in fungal cell membranes. Binding to ergosterol causes the formation of pores in the membrane. Potassium and other cellular constituents leak from the pores causing cell death.

Signs and SymptomsDiarrhea (including one case of bloody diarrhea), nausea, vomiting, gastrointestinal upset/disturbances. Rash, including urticaria has been reported rarely. (L1873)
Soil Adsorption/Mobility · 1 件

The Koc of nystatin is estimated as 170(SRC), using a water solubility of 3.60X10+2 mg/L at 25 °C(1)and a regression-derived equation(2). According to a classification scheme(3), this estimated Koc value suggests that nystatin is expected to have moderate mobility in soil.

Natural Pollution SourcesNystatin is produced from Streptomyces noursei, S. aureus, and other Streptomyces species(1).
Human Toxicity Excerpts · 2 件

/CASE REPORTS/ ...Few cases of local allergic reactions have been reported.

/EPIDEMIOLOGY STUDIES/ Prescription frequencies in the first trimester were compared for miconazole, clotrimazole, nystatin, candicidin, aminacrine compounds, and metronidazole before deliveries involving congenital anomalies versus those not linked to congenital anomalies. Prescriptions before spontaneous abortions were compared with those in the first trimester of deliveries and with those before legal abortions. No statistically significant association was observed with any of these agents for the overall frequency of birth defects or for specific birth defects analyzed (cardiovascular defects, oral clefts, and spina bifida). Two hundred fifty miconazole exposures among 4264 spontaneous abortions, compared with 2236 in the first trimester of 55,736 deliveries, provided an estimated relative risk of 1.4 (95% confidence limits 1.2-1.5). One hundred twelve treatments with clotrimazole among the spontaneous abortions, compared with 1086 among the deliveries, provided a relative risk of 1.4 (95% confidence limits 1.1-1.6). In contrast, large numbers of exposures to nystatin and aminacrine compounds did not show this association, suggesting that spontaneous abortions are caused by the imidazole agents miconazole and clotrimazole rather than the condition being treated. Because many associations were examined without previous hypotheses, and because the data were inadequate to show an elevated risk for clotrimazole when comparing spontaneous with legal abortion exposures, these findings are considered to be a signal for further studies rather than definitive in themselves.

Artificial Pollution SourcesNystatin's production and use as an antibiotic, experimental teratogen(1), feed additive(2), and a veterinarian medication(3) may result in its release to the environment through various waste streams(SRC).
Environmental BiodegradationAEROBIC: Nystatin, present at 2.56 mg/L, degraded 4% after 28 days(1) using Closed Bottle Test(1) which employs a sewage inoculm(2).
Carcinogen ClassificationNo indication of carcinogenicity to humans (not listed by IARC).

Pharmacodynamics · 1 件

Nystatin is an antifungal that is both fungistatic and fungicidal in vitro against a wide variety of yeasts and yeast-like fungi. It exerts its antifungal effects via disruption of the fungal cell membrane. Resistance to nystatin is minimal in _Candida albicans_, but tends to develop in other species of _Candida_. Nystatin carries no significant activity against bacteria, protozoa, or viruses. It carries significant systemic toxicity and is currently unavailable in a formula appropriate for systemic use - its efficacy is currently restricted, therefore, to topical, oral, and gastrointestinal infections.

Mechanism of Action · 3 件

Nystatin is a channel-forming ionophore, meaning it exerts its therapeutic effect via formation of a membrane-spanning pore in the fungal plasma membrane. The formation of this pore results in a change in membrane permeability that allows for leakage of intracellular contents and the subsequent disruption of electrochemical gradients necessary for proper cell function. Selectivity for fungal cells over mammalian cells is due to nystatin’s greater binding affinity for ergosterol, a key sterol found in fungal cell walls, as opposed to its mammalian counterpart, cholesterol.

Nystatin exerts its antifungal activity by binding to sterols in the fungal cell membrane. The drug is not active against organisms (e.g., bacteria) that do not contain sterols in their cell membrane. As a result of this binding, the membrane is no longer able to function as a selective barrier, and potassium and other cellular constituents are lost.

Absorption, Distribution and Excretion · 6 件

Systemic absorption of nystatin is minimal following oral administration, and no detectable plasma concentrations are attained following topical or vaginal administration.

The majority of orally administered nystatin is eliminated unchanged in the feces.

Nystatin is not absorbed into the systemic circulation and thus does not undergo distribution.

Nystatin penetrates eye poorly.

Nystatin is poorly absorbed from the GI tract, and detectable blood concentrations are not obtained after usual doses. Following oral administration, nystatin is excreted almost entirely in feces as unchanged drug.

In healthy individuals, mean salivary nystatin concentrations in excess of those required in vitro for growth inhibition of clinically important Candida persist for approximately 2 hours after the beginning of oral dissolution of two nystatin lozenges (400,000 units) administered simultaneously.

説明Nystatin was found in the mycelium of Streptomyces noursei in 1950 and produced on an industrial scale by Squibb & Sons Co. in 1954. This antibiotic was used orally and topically as the first clinically applied polyene macrolide with antifungal properties. Nystatin shows activity against Candida and filamentous fungi and is used to treat Candida infections of the mouth, digestive organs, and vagina. The application of nystatin in combination with gentamicin and vancomycin to sterilize the gut in perioperation of bone-marrow transplantation has been developed recently.

使用Polyene antifungal antibiotic complex containing 3 biologically active components, A1, A2, A3. Increases the permeability of the cell membrane of sensitive fungi by binding to sterols. Cell culture tested. Growth promotant. Nystatin A1 is shown.

一般的な説明Nystatin (Mycostatin) is a polyene antibiotic that was first isolated in 1951 from a strain of the actinomycete Streptomyces noursei by Hazen and Brown.33 It occurs as a yellow to light tan powder. Nystatin is very slightly soluble in water and sparingly soluble in organic solvents. The compound is unstable to moisture, heat, and light. The aglycone portion of nystatin is called nystatinolide. The complete structure of nystatin has been determined by chemical degradation and x-ray crystallography.35 Nystatin is not absorbed systemically when administered by the oral route. It is nearly insoluble under all conditions. It is also too toxic to be administered parenterally. Hence, it is used only as a topical agent. Nystatin is a valuable agent for the treatment of local and gastrointestinal monilial infections caused by C. albicans and other Candida species. For the treatment of cutaneous and mucocutaneous candidiasis, it is supplied as a cream, an ointment, and a powder. Vaginal tablets are available for the control of vaginal candidiasis. Oral tablets and troches are used in the treatment of gastrointestinal and oral candidiasis.
