Mechanism of Action · 6 件

Amobarbital (like all barbiturates) works by binding to the GABAA receptor at either the alpha or the beta sub unit. These are binding sites that are distinct from GABA itself and also distinct from the benzodiazepine binding site. Like benzodiazepines, barbiturates potentiate the effect of GABA at this receptor. This GABAA receptor binding decreases input resistance, depresses burst and tonic firing, especially in ventrobasal and intralaminar neurons, while at the same time increasing burst duration and mean conductance at individual chloride channels; this increases both the amplitude and decay time of inhibitory postsynaptic currents. In addition to this GABA-ergic effect, barbiturates also block the AMPA receptor, a subtype of glutamate receptor. Glutamate is the principal excitatory neurotransmitter in the mammalian CNS. Amobarbital also appears to bind neuronal nicotinic acetylcholine receptors.

The exact mechanism(s) by which barbiturates exert their effect on the CNS, has not been fully elucidated. However, it is believed that such effects are related, at least partially, to the drugs' ability to enhance the activity of gamma-aminobutyric acid (GABA), the principal inhibitory neurotransmitter in the CNS, by altering inhibitory synaptic transmissions that are mediated by GABAA receptors. /Barbiturates General Statement/

Although the drugs act throughout the CNS, a site of particular sensitivity is the polysynaptic midbrain reticular formation which is concerned with the arousal mechanism. Barbiturates induce an imbalance in central inhibitory and facilitatory mechanisms influencing the cerebral cortex and the reticular formation. The significance of the effect of barbiturates on neurotransmitters is unclear. It appears that the drugs decrease the excitability of both presynaptic and postsynaptic membranes. It has not been determined which of the various actions of barbiturates at cellular and synaptic levels are responsible for their sedative and hypnotic effects. /Barbiturates General Statement/

Relatively low doses of the barbiturates depress the sensory cortex, decrease motor activity, and produce sedation and drowsiness. In some patients, however, drowsiness may be preceded by a period of transient elation, confusion, euphoria, or excitement, especially after subhypnotic doses of aprobarbital, pentobarbital, or secobarbital. /Barbiturates General Statement/

Larger doses distort judgment, cloud perception, suppress motor activity, and produce drowsiness and sleep. Still larger doses induce anesthesia. Barbiturate-induced sleep differs from physiologic sleep. Barbiturates reduce the rapid eye movement (REM) or dreaming stage of sleep. Stages III and IV sleep are also decreased. Although tolerance develops to the REM-suppressant effects during chronic administration, REM rebound occurs when the drugs are withdrawn, and the patient may experience markedly increased dreaming, nightmares, and/or insomnia. /Barbiturates General Statement/

Amines are chemical bases. They neutralize acids to form salts plus water. These acid-base reactions are exothermic. The amount of heat that is evolved per mole of amine in a neutralization is largely independent of the strength of the amine as a base. Amines may be incompatible with isocyanates, halogenated organics, peroxides, phenols (acidic), epoxides, anhydrides, and acid halides. Flammable gaseous hydrogen is generated by amines in combination with strong reducing agents, such as hydrides.

To determine the effect of human growth hormone (somatropin) on human metabolism, 6 somatropin deficient children were given single oral doses of amobarbital sodium 3-5 mg 3 times/wk. The half-life of amobarbital sodium rose from 13.89 to 22.75 hr, vol of distribution was unchanged, and clearance fell. Results indicate that somatropin slows the metabolism of amobarbital sodium, probably through an effect on hepatic microsomal drug oxidizing system. /Amobarbital sodium/

Concurrent administration of disulfiram with barbiturates may result in inhibition of metabolism of the barbiturates and an increased incidence of barbiturate toxic effects. /Barbiturates/

Concurrent use with barbiturates may result in increased metabolism, leading to decreased serum concentrations and reduced elimination half-lives of carbamazepine or succinimide anticonvulsants because of induction of hepatic microsomal enzyme activity. Monitoring of serum concentrations as a guide to dosage is recommended, especially when carbamazepine or a succinimide anticonvulsant is added to or withdrawn from an existing regimen. /Barbiturates/

Concurrent use with barbiturates, especially phenobarbital, may induce microsomal metabolism to increase formation of alkylating metabolites of cyclophosphamide, thereby reducing the half-life and increasing the leukopenic activity of cyclophosphamide. /Barbiturates/

Because barbiturates are distributed into the milk of nursing women, nursing should be discontinued if nursing infants of barbiturate-treated women exhibit signs of toxicity. /Barbiturates General Statement/

Antibiotic-associated pseudomembranous colitis, caused by toxin-producing clostridia (e.g., C. difficile), has been reported with the use of nearly all anti-infectives, including IV vancomycin, and should be considered in the differential diagnosis of patients who present with diarrhea subsequent to administration of anti-infectives. Pseudomembranous colitis may occur during or following discontinuance of anti-infective therapy and ranges in severity from mild to life-threatening. Mild cases of colitis may respond to discontinuance of the drug alone, but diagnosis and management of moderate to severe cases should include appropriate bacteriologic studies and treatment with fluid, electrolyte, and protein supplementation as indicated. If colitis is moderate to severe and is not relieved by discontinuance of the drug, appropriate anti-infective therapy (e.g., metronidazole) should be administered. Nausea also has been reported in patients receiving vancomycin therapy.

The /barbiturates/ are distributed into milk in concentrations much lower than those in plasma. /Barbiturates General Statement/

Human Toxicity Excerpts · 5 件

Hypersensitivity reactions to barbiturates include urticaria, angioedema, morbilliform or scarlatiniform rash, fever, serum sickness, erythema multiforme, or Stevens-Johnson syndrome. Hypersensitivity reactions are most likely to occur in patients with asthma, urticaria, or angioedema. Rarely, photosensitivity reactions have been reported. Phenobarbital, metharbital, and mephobarbital appear to cause rashes more frequently than do other barbiturates. Exfoliative dermatitis, which may be accompanied by hepatitis and jaundice, has occurred rarely and has resulted in a few fatalities. Rarely, cutaneous reactions have preceded the development of barbiturate-induced systemic lupus erythematosus. Adverse cutaneous reactions may proceed to an irreversible stage even though the medication has been discontinued, because barbiturates are slowly metabolized and excreted. /Barbiturates General Statement/

Daily admin of 600-800 mg of amobarbital ... for approx 8 wk will produce some degree of physical dependence. ... Symptoms of barbiturate dependence are similar to those of chronic alcoholism, but can be more severe than those associated with alcohol or opiate abstinence syndrome & may result in fatalities.

/HUMAN EXPOSURE STUDIES/ Eleven right-handed subjects were administered a dichotic listening test to assess cerebral hemispheric dominance, both under a drug-free condition and following systemic injection of the barbiturate, amobarbital. The finding of increased right ear advantage during the drug period reflects increased left cerebral dominance, and suggests that amobarbital may have an asymmetric, pharmacological effect on the two cerebral hemispheres.

/LABORATORY ANIMALS: Chronic Exposure or Carcinogenicity/ Thirty male F344/NRCFCRC rats were injected with 75 mg DENA (dissolved in saline) intraperitoneally at 4 weeks of age. Amobarbital (AB), 500 ppm, was given in the drinking water from week 6 to 78. This group was compared to a N-nitrosodiethylamine only treated group (30 animals). There were no significant differences between the groups in survival, weight gain, or induced tumors. At 52 weeks, when 1/2 of the animals were autopsied, 1/15 amobarbital and N-nitrosodiethylamine treated animals with hepatocellular tumors compared with 2/15 for the N-nitrosodiethylamine alone treated group. At 78 weeks, 8/15 of the amobarbital and N-nitrosodiethylamine treated animals had hepatocellular tumors compared with 10/15 for the N-nitrosodiethylamine alone treated group. There were no increases in any other tumors over the N-nitrosodiethylamine group. Additionally, Amobarbital was tested alone. Twenty-five animals were injected with saline (5 ml/kg body weight) at 4 weeks of age and given 500 ppm amobarbital in the drinking water from 6 to 78 weeks. They were compared to the saline injected only group. There were no effects of amobarbital on survival or body weight for the 78 weeks of the experiment. There were no hepatocellular tumors in the amobarbital or the saline groups at 52 or at 78 weeks. There were no significant differences in the number of tumors at other sites.

/LABORATORY ANIMALS: Chronic Exposure or Carcinogenicity/ Tumor-promoting abilities of four barbiturates, phenobarbital [(PB) CAS: 50-06-6], amobarbital [(AB) CAS: 57-43-2], barbital sodium [(BB) CAS: 144-02-5], and barbituric acid [(BA) CAS: 67-52-7], on the development of neoplasms in livers and other organs of rats following initiation with N-nitrosodiethylamine [(DENA) CAS: 55-18-5] were compared. Four-week-old F344/NCr male rats were given a single ip injection of 75 mg DENA/kg body weight. Beginning 2 weeks later, they were given either tap water (group 1) or drinking water containing 500 ppm of PB (group 2), the sodium salt of BB (group 3), AB (group 4), or BA (group 5) for the remaining experimental period. Control groups (groups 6-10) received an ip injection of saline alone and 2 weeks later were given either tap water or drinking water containing barbiturates as listed above. Animals were sacrificed at either 52 weeks or 78 weeks. None of the barbiturates altered the growth and survival of animals. PB and BB increased liver weights and significantly enhanced the development of hepatocellular foci and hepatocellular adenomas at 52 weeks and hepatocellular foci, hepatocellular adenomas, and trabecular carcinomas at 78 weeks in DENA-treated rats. No such enhancing effects were observed with AB or BA. PB or BB did not significantly enhance the incidence of nonhepatic tumors at 52 weeks. However, at 78 weeks BB significantly enhanced the development of renal tubular adenomas and carcinomas, while PB enhanced the development of thyroid follicular cell neoplasms in DENA-treated rats. These results clearly showed that barbiturates exhibited structure-promoting activity relationships and that their promoting abilities were not restricted to liver alone. Substitution of both hydrogen atoms at the C-5 position of the pyrimidine ring by alkyl or aryl groups appears to be essential but not sufficient for tumor-promoting activity of barbiturates.

Biological Half-Life · 3 件

Following bolus iv administration, plasma concentrations of amobarbital decline in a biphasic manner with a half-life of about 40 minutes for the first phase and 20-25 hours for the second phase, although the second phase half-life has ranged from 14-42 hours in individual patients.

The half-life of amobarbital was investigated in 36 unrelated subjects; half-life (23.8 hr) appeared to be normally distributed.

Following im injection of amylobarbitone sodium 200 mg to mildly hypertensive women in labor 0.7 to 3.5 hr before delivery, the plasma-half-life of amylobarbitone was 2.5 times as long in the neonates as in the mothers. /Amobarbital sodium/

Metabolism/Metabolites · 5 件

Amobarbital is metabolized by the liver via penultimate oxidation of the 3-methylbutyl substituent to form a tertiary alcohol, hydroxyamobarbital, which is an inactive metabolite. About 40-50% of an oral hypnotic dose of amobarbital is excreted in urine as hydroxyamobarbital and its glucuronide conjugates. Less than 1% of an oral hypnotic dose of the drug is excreted in urine unchanged. Conjugates of hydroxyamobarbital excreted in feces or urine and/or further oxidation products not yet identified may account for the remainder of the dose.

Following admin of radioactively labelled amylobarbitone to two healthy subjects ... less than 50% of the dose was identified as 3'-hydroxyamylobarbitone. A second main metabolite was identified as N-hydroxyamylobarbitone and was found to account for up to 30% of the dose.

Relative proportion of amobarbital metabolites in urine is highly variable and observations of plasma half-life give no indication of this variability. A valid estimate of a given person's metabolite pattern can be obtained by studying single urine specimen in postdistributive phase. Two metabolites were measured in urine specimen in the postdistributive phase. The 2 metabolites were 3'-hydroxyamobarbital as product of side chain hydroxylation and N-beta-d-glycopyranosyl amobarbital, a glucose conjugate.

N-Hydroxyamylobarbitone was synthesized and its occurrence as a urinary metabolite was studied in 13 subjects taking 200 mg sodium amobarbital. It was concluded that the excretion of free N-hydroxyamylobarbitone is not significant in the metabolism of amobarbital sodium. /Amobarbital sodium/

MeSH Pharmacological Classification · 2 件

Drugs used to induce drowsiness or sleep or to reduce psychological excitement or anxiety.

Substances that do not act as agonists or antagonists but do affect the GAMMA-AMINOBUTYRIC ACID receptor-ionophore complex. GABA-A receptors (RECEPTORS, GABA-A) appear to have at least three allosteric sites at which modulators act: a site at which BENZODIAZEPINES act by increasing the opening frequency of GAMMA-AMINOBUTYRIC ACID-activated chloride channels; a site at which BARBITURATES act to prolong the duration of channel opening; and a site at which some steroids may act. GENERAL ANESTHETICS probably act at least partly by potentiating GABAergic responses, but they are not included here.

Following oral or rectal admin, the onset of action varies from 10-30 min for ... amobarbital ... Following iv admin of the sodium salts of ... amobarbital, the onset of action ranges from almost immediately for pentobarbital ... .

About 40 to 60% of amylobarbitone is bound to plasma proteins.

Following admin of radioactively labelled amylobarbitone to 2 healthy subjects 79 to 92% was recovered in urine in 6 days and only 4 to 5% in the feces. Unchanged drug was practically absent from both urine and feces.

Fluid & tissue specimens collected from 30 subjects at autopsy were assayed for amylobarbitone (amobarbital), butobarbitone (butethal), pentobarbitone (pentobarbital), quinalbarbitone (secobarbital) and the corresponding hydroxylated metabolites by GLC. Where one barbiturate was ingested, an inverse relationship between lipid solubility of the drug and the distribution in fluids and tissues was observed. In most cases the liver, and in the remainder the spleen, contained the highest concn of barbiturate. Bile concn were often in excess of those in the corresponding liver. The metabolites of the 4 sedative barbiturates were usually present in lower amounts than the parent drugs in the fluids and tissues of most subjects, but urine often contained much higher concn of metabolites, sometimes exceeding that of the parent drug in the liver. Admin of 2 or more barbiturates together did not appear to affect the distribution and metabolism of the individual drugs.

Methods of Manufacturing · 3 件

A typical method starts with monochloroacetic acid, which is treated with sodium cyanide to form cyanoacetic acid; the latter is reacted with hydrochloric acid in the presence of alcohol, yielding the diethyl ester of malonic acid. This ester, in absolute alcohol solution, is treated with the theoretical quantity of metallic sodium to replace one hydrogen of the CH2 group, then a slight excess of the theoretical amount of an ethylating agent, such as ethyl bromide, is added. The second hydrogen is replaced similarly, using isopentyl bromide as the alkylating agent. The diethyl ester of ethyl isopentyl malonic acid thus obtained is heated in an alcoholic solution, in the presence of sodium, with urea. Sodium amobarbital is formed, from which amobarbital is liberated with HCl. The alkylation of the CH2 group of the malonic ester ... may be done in two stages, introducing one alkyl group at a time.

By reacting amobarbital with a solution containing a chemically equivalent quantity of sodium hydroxide or sodium carbonate, evaporating to dryness, and crystallizing the residue from a solution in a suitable solvent such as alcohol. /Amobarbital sodium/

The 5,5-disubstituted barbituric acids are still synthesized by the classical condensation of substituted malonic esters, malonic amides, or cyanoacetic esters with urea, thiourea, guanidine, or dicyanodiamide, followed by the hydrolysis of the resulting imino- or cyanobarbiturate. /5,5-Disubstituted barbituric acids/

補足説明

定義ChEBI: A member of the class of barbiturates that is pyrimidine-2,4,6(1H,3H,5H)-trione substituted by a 3-methylbutyl and an ethyl group at position 5. Amobarbital has been shown to exhibit sedative and hy notic properties.

/LABORATORY ANIMALS: Chronic Exposure or Carcinogenicity/ The effects of phenobarbital (PB) and amobarbital (AB) on the rate of development of hepatocarcinogenesis induced by N-nitrosodiethylamine (DEN) were studied in mice. Groups of 40 B6C3F1 male mice were injected i.p. at 15 days of age with 5 micrograms DEN/g body wt. Beginning at 4 weeks of age, DEN treated groups were given either normal drinking water or water containing either 0.05% PB or AB for up to 36 weeks. DEN alone induced multiple focal hepatic lesions including hepatocellular foci, hepatocellular adenomas and trabecular carcinomas. Subsequent exposure to PB had a suppressing effect on DEN-induced hepatocarcinogenesis. Hepatocellular foci in PB-exposed mice were significantly smaller in size (area) and fewer in number throughout the study. Also, PB treatment either prolonged the latency period or significantly slowed the rate at which hepatocellular tumors developed in these mice. No such effects were seen in AB-exposed mice; AB neither inhibited nor promoted the development of focal hepatic lesions in DEN-pretreated mice. Possible mechanisms responsible for the inhibition of DEN-induced hepatocarcinogenesis include the feminizing effects of perinatal administration of PB.
