Iron supplementation typically results in increases in serum iron, transferrin-bound iron, and iron-stored in the form of ferritin in hepatocytes and macrophages. The available iron is usually used in bone marrow for the synthesis of hemoglobin.

Mechanism of Action · 1 件

The usage of ferric pyrophosphate is based on the strong complex formation between these two species. Besides, the capacity of pyrophosphate to trigger iron removal from transferrin, enhance iron transfer from transferrin to ferritin and promote iron exchange between transferrin molecules. These properties make it a very suitable compound for parenteral administration, iron delivery into circulation and incorporation into hemoglobin.

...The aim of the present studies was to compare Fe absorption of micronised, dispersible ferric pyrophosphate (Sunactive Fe trade mark ) with that of ferrous sulfate in an infant cereal and a yogurt drink. Two separate Fe absorption studies were made in adult women (10 women/study). Fe absorption was based on the erythrocyte incorporation of stable isotopes ((57)Fe and (58)Fe) 14 days after the intake of labelled test meals of infant cereal (study 1) or yogurt drink (study 2). Each test meal was fortified with 5 mg Fe as ferrous sulfate or micronised, dispersible ferric pyrophosphate. Results are presented as geometric means. There was no statistically significant difference between Fe absorption from micronised, dispersible ferric pyrophosphate- and ferrous sulfate-fortified infant cereal (3.4 and 4.1% respectively; P=0.24) and yogurt drink (3.9 and 4.2% respectively; P=0.72).

化学的特性Ferric pyrophosphate (iron(III)pyrophosphate, Fe4(P207)3*xH2O) is a tan or yellowish-white odorless powder or Soluble crystals. It is insoluble in water but is soluble in mineral acids. It is prepared by reacting sodium pyrophosphate with ferric citrate. Ferric pyrophosphate is an important kind of compounds insoluble in water and also poorly soluble in dilute acid. It does not modify organoleptic characteristics of foods because of its white colour and low reactivity, making it suitable for many difficult-to-fortify food vehicles, such as salt, cereals, infant formulas, and rice. It has been demonstrated to be an effective iron fortificant from efficacy studies. However, the poor solubility of ferric pyrophosphate in gastric juice limits its bioavailability, which furthemore reduces its nutritional value. The relative bioavailability value (RBV) of ferric pyrophosphate was reported to be only about 50% from human studies.

SRP: The most favorable course of action is to use an alternative chemical product with less inherent propensity for occupational exposure or environmental contamination. Recycle any unused portion of the material for its approved use or return it to the manufacturer or supplier. Ultimate disposal of the chemical must consider: the material's impact on air quality; potential migration in soil or water; effects on animal, aquatic, and plant life; and conformance with environmental and public health regulations.

SRP: The scientific literature for the use of contact lenses in industry is conflicting. The benefit or detrimental effects of wearing contact lenses depend not only upon the substance, but also on factors including the form of the substance, characteristics and duration of the exposure, the uses of other eye protection equipment, and the hygiene of the lenses. However, there may be individual substances whose irritating or corrosive properties are such that the wearing of contact lenses would be harmful to the eye. In those specific cases, contact lenses should not be worn. In any event, the usual eye protection equipment should be worn even when contact lenses are in place.

...Mice were treated orally with ciprofloxacin (50 mg/kg) alone, the iron compound (ferrous sulphate, sodium ferrous citrate or ferric pyrophosphate; 50 mg elemental iron/kg) alone, ciprofloxacin with each iron compound or ciprofloxacin in combination with each iron compound and ascorbic acid (250 mg/kg). The maximum serum concentration of ciprofloxacin was significantly (P<0.01) reduced from 1.15+/-0.11 ug/mL (ciprofloxacin alone) to 0.17+/-0.01, 0.27+/-0.01 or 0.28+/-0.02 ug/mL, respectively, when ferrous sulphate, sodium ferrous citrate or ferric pyrophosphate was administered along with ciprofloxacin. The addition of ascorbic acid did not affect the inhibitory effects of each iron compound on the absorption of ciprofloxacin. Ciprofloxacin did not affect the variation of serum iron levels after administration of each iron compound. The addition of ascorbic acid significantly (P<0.01) enhanced the increase in serum iron concentration after administration of sodium ferrous citrate, showing an increase from 270+/-6 ug/dL to 463+/-11 ug/dL compared with an increase from 248+/-8 ug/dL to 394+/-18 ug/dL after administration of sodium ferrous citrate alone. Ascorbic acid also caused a significant (P<0.01) increase in serum iron concentration from 261+/-16 ug/dL to 360+/-12 ug/dL after administration of ferric pyrophosphate, although it did not affect the levels after ferrous sulphate administration.

Maintain an open airway and assist ventilation if necessary. Treat shock caused by hemorrhagic gastrointestinitis aggressively with intravenous crystalloid fluids, and replace blood if needed. Patients are often markedly hypovolemic owing to gastrointestinal losses and third spacing of fluids into the intestinal wall and interstitial space. Treat coma, seizures, and metabolic acidosis if they occur. For seriously intoxicated victims (eg, shock, severe acidosis, and/or serum iron > 500-600 mcg/dL) administer deferoxamine. Monitor the urine for the urine for the characteristic orange or pink deferoxamine-iron complex. Therapy may be stopped when the urine returns to normal or when the serum iron level decreases to the normal range. Prolonged deferoxamine has been associated with adult respiratory distress syndrome and Yersinia sepsis.

Activated charcoal is not effective. Ipecac is not recommended, because it can aggravate iron-induced gastrointestinal irritation and interfere with whole bowel irrigation. Consider gastric lavage if product was a liquid formulation or tablets were chewed. Do-not use phosphate-containing solutions for lavage; these may result in life-threatening hypernatremia, hyperphosphatemia, and hypocalcemia. Bicarbonate and deferoxamine lavage are of doubtful efficacy. Deferoxamine lavage is not effective and may enhance iron absorption. Whole-bowel irrigation is very effective for ingested tablets and may be considered first-line treatment, especially if large numbers of tablets are visible on plain abdominal x-ray. Massive ingestions may lead to concretions or bezoars. Repeated or prolonged whole-bowel irrigation may remove tablets. Endoscopy or surgical gastrotomy is rarely required.

Hemodialysis and hemoperfusion are not effective for removing iron but may be necessary to remove iron-deferoxamine complex in patients with renal failure. Exchange transfusion is occasionally used for massive pediatric ingestions but is of questionable efficacy.

説明Iron, which is the chief constituent of Earth’s core, is the most abundant element in Earth as a whole (about 35 percent) . Ferric pyrophosphate is an iron replacement product, is a mixed-ligand iron complex in which iron (III) is bound to pyrophosphate and citrate. It is widely used to fortify cereal flours and infant cereals, dispersible ferric pyrophosphate is as well absorbed as ferrous sulfate in adults. pyrophosphate ions participate in the ion transfer processes. These ions, as chelating agents, would release iron from transferrin and form a ferric pyrophosphate complex, and that pyrophosphate alone has the ability to mediate iron exchange from transferring to ferritin. Ferric pyrophosphate citrate should only be added to the bicarbonate concentrate; do not add to acid concentrate mixtures. Ferric pyrophosphate citrate is not intended for use in patients receiving peritoneal dialysis; it has not been studied in patients receiving home hemodialysis.
