The efficacy of xanomeline in the treatment of schizophrenia is thought to be due to its agonist activity at muscarinic acetylcholine receptors M1 and M4 in the central nervous system. It binds to muscarinic receptors M1 to M5 with comparable affinity and exhibits relatively higher agonist activity at the M1 and M4 receptors.
Xanomeline is extensively metabolized, both by CYP450 enzymes - including CYP2D6, CYP2B6, CYP1A2, CYP2C9, and CYP2C19 - as well as flavin monooxygenases (FMO1 and FMO3). Unchanged parent drug accounts for <0.01% of drug product excreted in the urine.

Drugs that mimic the effects of parasympathetic nervous system activity. Included here are drugs that directly stimulate muscarinic receptors and drugs that potentiate cholinergic activity, usually by slowing the breakdown of acetylcholine (CHOLINESTERASE INHIBITORS). Drugs that stimulate both sympathetic and parasympathetic postganglionic neurons (GANGLIONIC STIMULANTS) are not included here.
A loosely defined grouping of drugs that have effects on psychological function. Here the psychotropic agents include the antidepressive agents, hallucinogens, and tranquilizing agents (including the antipsychotics and anti-anxiety agents).
Drugs that bind to and activate muscarinic cholinergic receptors (RECEPTORS, MUSCARINIC). Muscarinic agonists are most commonly used when it is desirable to increase smooth muscle tone, especially in the GI tract, urinary bladder and the eye. They may also be used to reduce heart rate.

131986-45-3DTXSID60157286CHEMBL21536CHEBI:10056Following oral administration of xanomeline, Tmax is reached in approximately 2 hours. Steady-state is reached in 3 to 5 days following the start of therapy.
Xanomeline and its metabolites are primarily eliminated in the urine. Approximately 78% of the total administered drug product was eliminated in the urine, mostly comprising metabolites (<0.01% was unchanged parent drug). Approximately 12% of the total administered drug product was eliminated in the feces.
The apparent volume of distribution following oral administration of xanomeline is approximately 10,800 liters.
The apparent clearance of xanomeline is 1950 L/hour. The renal clearance of xanomeline is 0.085 L/hour.
説明Xanomeline is used as a potent and selective muscarinic type 1 and type 4 (M1/M4) receptor agonist to increase neuronal excitability. Xanomeline is used in studies of neurological disorders such as Alzheimer's disease and schizophrenia. However, development of the drug was halted due to severe cholinergic adverse effects. Currently, xanomeline-trospium (KarXT) (a peripheral cholinergic antagonist) has been explored for the treatment of schizophrenia and a New Drug Application has been filed with the FDA, with a decision on whether it will be approved or not expected on 26 September 2024.
