Table: Potential Drug Interactions with Hydrochlorothiazide Tablet [Table#4429]

Lithium: Generally should not be given with diuretics. Diuretic agents reduce the renal clearance of lithium and add a high risk of lithium toxicity. Refer to the package insert for lithium preparations before use of such preparations with hydrochlorothiazide.

In some patients, the administration of a non-steroidal anti-inflammatory agent can reduce the diuretic, natriuretic, and antihypertensive effects of loop, potassium-sparing and thiazide diuretics. Therefore, when hydrochlorothiazide and non-steroidal anti-inflammatory agents are used concomitantly, the patient should be observed closely to determine if the desired effect of the diuretic is obtained.

The primary object of this fundamental research was to survey the synergistic cardiovascular effects of iptakalim, a novel ATP-sensitive potassium channel (K(ATP)) opener, and clinical first-line antihypertensive drugs, such as calcium antagonists, thiazide diuretics and beta receptor blockers by a 2 x 2 factorial-design experiment. It would provide a theoretical basis for the development of new combined antihypertensive therapy program after iptakalim is applied to the clinic. Amlodipine besylate, hydrochlorothiazide and propranolol were chosen as clinical first-line antihypertensive drugs. Blood pressure, heart rate (HR) and cardiac functions were observed in anesthetized normal rats by an eight-channel physiological recorder. The results showed that iptakalim monotherapy in a low dose could produce significant antihypertensive effect. There was no interaction between iptakalim and amlodipine on the maximal changes of systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial blood pressure (MABP), the left ventricular systolic pressure (LVSP), and the left ventricular end-diastolic pressure (LVEDP) (P > 0.05). However, the effects of combination iptakalim/amlodipine on the maximal changes of SBP, DBP, MABP, LVSP and LVEDP were more obvious than those of iptakalim or amlodipine monotherapy. And there was strong positive interaction between iptakalim and amlodipine on the maximal changes of HR (P>0.05). According to the maximal changes of DBP, MABP, LVSP and LVEDP (P < 0.05) of combination iptakalim with hydrochlorothiazide, there was strong positive interaction between them. But there was no interaction between iptakalim and hydrochlorothiazide on the maximal drop of SBP and HR (P > 0.05). According to the maximal drops of DBP, MABP of combination iptakalim with propranolol, there was strong positive interaction between them (P < 0.05). But there was no interaction between iptakalim and propranolol on the maximal changes of SBP, LVSP, LVEDP and H

Environmental Fate · 3건

TERRESTRIAL FATE: Based on a classification scheme(1), an estimated Koc value of 12(SRC), determined from a structure estimation method(2), indicates that hydrochlorothiazide is expected to have very high mobility in soil(SRC). The pKa values of hydrochlorothiazide are 7.9 and 9.2(3), indicating that this compound will exist as a zwitterion in the environment and zwitterion absorption is greatly influenced by substrate pH(4). Volatilization of hydrochlorothiazide from moist soil surfaces is not expected to be an important fate process(SRC) given an estimated Henry's Law constant of 4.4X10-12 atm-cu m/mole(SRC), developed using a fragment constant estimation method(3). Hydrochlorothiazide is not expected to volatilize from dry soil surfaces(SRC) based upon an estimated vapor pressure of 1.8X10-10 mm Hg at 25 °C(SRC), determined from a fragment constant method(2). Hydrochlorothiazide has been classified as not readily biodegradable(5).

AQUATIC FATE: Based on a classification scheme(1), an estimated Koc value of 12(SRC), determined from a structure estimation method(2), indicates that hydrochlorothiazide is not expected to adsorb to suspended solids and sediment(SRC). Reported pKa values of 7.9 and 9.2(3) indicate hydrochlorothiazide will exist as a zwitterion at pH values of 5 to 9 and, therefore, volatilization from water surfaces is not expected to be an important fate process as hydrochlorothiazole will exist as a neutral compound under environmental conditions (pH 5 to 9)(4). According to a classification scheme(5), an estimated BCF of 3(SRC), from its log Kow of -0.07(6) and a regression-derived equation(2), suggests the potential for bioconcentration in aquatic organisms is low(SRC). Hydrochlorothiazide has been classified as not readily biodegradable(8).

ATMOSPHERIC FATE: According to a model of gas/particle partitioning of semivolatile organic compounds in the atmosphere(1), hydrochlorothiazide, which has an estimated vapor pressure of 1.8X10-10 mm Hg at 25 °C((SRC), determined from a fragment constant method(2), is expected to exist solely in the particulate phase in the ambient atmosphere. Particulate-phase hydrochlorothiazide may be removed from the air by wet and dry deposition(SRC). Hydrochlorothiazide absorbs UV light at wavelengths >310 nm(3) and, therefore, may be susceptible to direct photolysis by sunlight(SRC).

Adverse Effects · 1건

Hydrochlorothiazide is generally well tolerated at low to moderate doses; however, adverse effects are dose-dependent and occur more frequently when daily doses exceed 25 mg.;Electrolyte Derangements;Hypokalemia, hyponatremia, hypomagnesemia, hypercalcemia, and hyperchloremic alkalosis may result from hydrochlorothiazide blocking the sodium-chloride cotransporter in the DCTs, as described earlier. Hyponatremia may be particularly pronounced in older adults, individuals with low solute intake, or those receiving concomitant diuretics. Electrolyte abnormalities are more common with higher doses or combination diuretic therapy.;Hyperglycemia;Hydrochlorothiazide has been associated with mild elevations in fasting plasma glucose. Although the mechanism is not fully elucidated, hypokalemia may impair pancreatic β-cell insulin secretion, contributing to dysglycemia. The effect is generally modest and appears to be dose-related, necessitating monitoring in patients with diabetes mellitus or impaired glucose tolerance.;Hyperuricemia and Gout;Hydrochlorothiazide can increase serum uric acid levels and precipitate gout flares, particularly in predisposed individuals. The mechanism involves volume contraction and enhanced proximal tubular urate reabsorption. Hyperuricemia typically develops within the first weeks of therapy initiation and is dose-dependent.;Dyslipidemia;Thiazide therapy has been associated with modest, dose-related increases in total cholesterol and triglyceride levels. These changes are generally small and may attenuate with long-term therapy.;Sulfa Allergy;Hydrochlorothiazide contains a sulfonamide moiety. Severe hypersensitivity reactions, including rash, urticaria, Stevens-Johnson syndrome, toxic epidermal necrolysis, and anaphylaxis, are rare but reported adverse effects. Although cross-reactivity between sulfonamide antibiotics and nonantibiotic sulfonamides, such as hydrochlorothiazide, is considered low, caution is advised in patients with a history of severe sulfonamide reactions. The absolute contraindication is generally limited to patients with a prior serious hypersensitivity to hydrochlorothiazide itself.;Ocular Disturbance;Rare cases of acute myopia and secondary angle-closure glaucoma have been reported, typically within days of therapy initiation. These events are thought to be related to idiosyncratic sulfonamide-associated choroidal effusion leading to anterior displacement of the lens–iris diaphragm. Immediate drug discontinuation is required if ocular symptoms develop.;Special Considerations; In patients with advanced liver disease, hydrochlorothiazide may precipitate severe hyponatremia, hypokalemia, or hepatic encephalopathy due to fluid and electrolyte shifts. Careful monitoring is required in this population.;Clinicians should also exercise caution during the perioperative period, as diuretic-induced volume depletion may increase the risk of intraoperative hypotension or postoperative acute kidney injury.;Drug-Drug Interactions;* Antidiabetic drugs: When hydrochlorothiazide is administered with antidiabetic drugs, including oral agents and insulin, dosage adjustment of the antidiabetic therapy may be required.;* Alcohol or narcotics: Concomitant use of alcohol, barbiturates, or narcotics with hydrochlorothiazide may potentiate orthostatic hypotension.;* Cholestyramine and colestipol resins: Cholestyramine or colestipol binds hydrochlorothiazide in the gastrointestinal tract, reducing its systemic absorption.

Pharmacodynamics · 1건

Hydrochlorothiazide prevents the reabsorption of sodium and water from the distal convoluted tubule, allowing for the increased elimination of water in the urine. Hydrochlorothiazide has a wide therapeutic window as dosing is individualized and can range from 25-100mg. Hydrochlorothiazide should be used with caution in patients with reduced kidney or liver function.

Mechanism of Action · 1건

Hydrochlorothiazide is transported from the circulation into epithelial cells of the distal convoluted tubule by the organic anion transporters OAT1, OAT3, and OAT4. From these cells, hydrochlorothiazide is transported to the lumen of the tubule by multidrug resistance associated protein 4 (MRP4). Normally, sodium is reabsorbed into epithelial cells of the distal convoluted tubule and pumped into the basolateral interstitium by a sodium-potassium ATPase, creating a concentration gradient between the epithelial cell and the distal convoluted tubule that promotes the reabsorption of water. Hydrochlorothiazide acts on the proximal region of the distal convoluted tubule, inhibiting reabsorption by the sodium-chloride symporter, also known as Solute Carrier Family 12 Member 3 (SLC12A3). Inhibition of SLC12A3 reduces the magnitude of the concentration gradient between the epithelial cell and distal convoluted tubule, reducing the reabsorption of water.

Biological Half-Life · 4건

The plasma half life of hydrochlorothiazide is 5.6-14.8h.

Based on determination of plasma drug concentrations over a period of at least 24 hours, the plasma half-life of hydrochlorothiazide reportedly ranges from 5.6-15 hours.

The bioavailability of hydrochlorothiazide from 50-mg oral tablet doses was examined in healthy male volunteers under fasting and nonfasting conditions. ... The pharmacokinetics of hydrochlorothiazide in plasma could be described in terms of a triexponential function, and the mean half-life determined from the 3 exponents were 1.0, 2.2, and 9.0 hr.

The radioactivity in /human/ plasma during the first 10 hr after oral administration /of hydrochlorothiazide (hct)/ declined with a fast phase but the levels of label thereafter suggested a slow phase. The existence of such a phase was verified in 1 subject given 75 mg hct orally. His plasma levels of hct (determined with gas-liquid chromatography) declined according to a 2-compartment model, the half-lives of the alpha-and beta-phases being 1.7 and 13.1 hr, respectively.
